Office Policy
- All narcotics prescriptions will not be refilled unless patient is seen by the doctor.
- NEVADA BAD CHECK LAW
The issuance of a check or checks without funds or with intent to defaud is punishable by imprisonment in the county jail for not more than 6 months or a find of not more than $500.00, or by both such fine and imprisonment. An issuance of such a check or checks in an amount of $100.00 or more, or by a person who previously has been convicted three times of this or a similar offence, is punishable by imprisonment in The State Prison for not less than one year not more than 10 years, or by a fine of not more than $10,000, or by both fine and imprisonment.

- There is a $25.00 charge on all Returned Checks.
- All Co-Payments are payable at time of service and are collected at time of check-in, unless prior arrangements have been made.
- Please give Receptionist change of address and all insurance information. We file insurance as a courtesy. Charges incurred are ultimately the patient responsibility.
- There is a fee for completion of all disability, government, and work release forms.
- Please allow up to three days for all prescription refills.
- There is a $25.00 charge on all Returned Checks.
